Online Services

We offer an online service (called Patient Access) that you can use 24/7 to:

  • Order repeat prescriptions
  • Cancel any appointment
  • Book some types of appointment
  • View some parts of your medical record
  • Change your address or phone numbers

For your safety and security, the system is protected by comprehensive security measures: you can only access the service after we have verified your identity and given you the required activation codes.

If you haven't yet received your activation codes from us, see below for "How to apply".

How to apply for online services

To protect your information, you need to apply for this service separately, and provide us with proof of your identity.

  1. The service is available to registered patients aged 16 and over. 
  2. Read the Guide to Online Services.
  3. Print and complete the Online Access Application Form - but don't sign it until the receptionist asks you. The form is also available from reception, in case you don't have a printer.
  4. Bring the form along with two forms of identification: one from each of the lists below. Only these documents are acceptable, so please ensure you bring the correct ones.
  5. The receptionist will check your identity and ask you to sign the form. You will be given a printout with the passwords and codes you need to complete the online registration for the service.
  6. Access the service using the link above, click Register and follow the instructions.

Acceptable identification documents

You need to bring one document from each list.

List A - must be currently valid

  • Passport (any country)
  • Photocard driving licence UK
  • EU national identity card
  • UK identity card for foreign nationals (ICFN)
  • UK ARC card (asylum seeker)
  • UK Armed Forces ID card
  • Proof of age card issued under the Proof of Age Standards Scheme (containing a unique reference number)
  • UK Biometric Residence Permit (BRP)
  • UK government issued certificate of identity
  • UK government issued stateless card

List B - must be originals and not printed from the internet

  • Bank statement *
  • Credit card account statement *
  • Vehicle insurance *
  • Home insurance *
  • Mortgage statement *
  • Birth Certificate
  • Adoption Certificate
  • Fire Brigade ID card
  • National 60 plus card
  • DBS enhanced disclosure certificate *
  • Firearm certificate (current)

* dated within the last year.
